Ø  Liability Release- notarized

Ø  Aspendale Assumption of Risk Form

All Sponsors must fill out the following forms:

Ø  Sponsor Registration Form

Ø  Liability Release – notarized (if minor)

Ø  Aspendale Assumption of Risk Form

Ø  Volunteer Application (or your church’s volunteer application)

The church needs to fill out the following forms:

Ø  Letter of recommendation for Sponsors

Ø  Church Registration form (Due April 8 for Early Registration)

Ø  A list of all children with their buddies (buddy slips are provided to help make this list)
